Who We Are

Precision Case Management is a private-pay psychiatric case management firm. Our mission is to improve the quality and value of a client's life by enhancing well-being, health, and independence. We believe the client and the family both need an advocate to help ensure necessary services and treatments are received.

Our focus is to:

• Understand the individual client's life situation and medical condition.

• Collaborate with the client's psychiatrist, physician, and mental health professionals to assist in maintaining an effective treatment plan.

• Help the client understand their condition and appreciate their role in the path toward recovery.

• Work with the family to promote a positive outcome.

Who We Serve

• Individuals and family members who need help in life while coping with mental illness.

• Many clients are referred by their mental health professional.

Services We Offer

• Working with both the client and family to develop a long-term plan of care with client independence as a goal.

• Coordinating the client's health care with their psychiatrist/psychologist and physicians to help ensure proper and timely medical services and treatment are received.

• Coordinating the client's economic assistance with public and private support agencies to help secure services and financial assistance.

• Helping with the planning and day-to-day decision making process of the client as well as assisting family members.
